Take an infant under 3 months to a doctor for any fever of 100.4°F (38°C) or higher. For older children, see a doctor if fever crosses 104°F (40°C), lasts more than 2-3 days, or comes with a stiff neck, rash, persistent vomiting, difficulty breathing, or unusual drowsiness.
A young infant's immune system cannot fight infection the way an older child's can, which is why any fever in a baby under 3 months needs same-day medical assessment. For children over 2, mild fevers under 102°F that respond to home care rarely need urgent attention on their own.
Difficulty breathing, a rash that does not fade when pressed, repeated vomiting, a bulging soft spot in infants, seizures, or a child who is unusually hard to wake up all warrant immediate evaluation even if the fever itself seems moderate.
Keep the child hydrated, dress them lightly, and use paracetamol at the correct weight-based dose if they are uncomfortable. Avoid combining fever medicines without a doctor's guidance, and avoid sponging with cold water or alcohol.
